My understanding is that the AC (5-10 miles) is pretty much the accepted standard.
Anything else can't be interpreted as more conservative and therefore would be harder to defend in court.

Yeah. You depart an IFR airport, go do your visual maneuvers on top and then go back and shoot the approach.
You don't want to cancel because then you gotta start from scratch. It's just more efficient.

Clearance limit
This sounds like you were filed to some fix (the limit of your clearance) where you expected to be "on top" and intended to cancel.
That's totally different than receiving an explicit "maintain VFR-on-top" clearance.
Just like the clearance to enter Class B airspace...

We (Atlas) don't have any flights into BRU this entire month.
I checked ORD to Europe and there's one flight this weekend that stops in JFK (26 hours for scheduled mx) and then continues on to Prestwick, Scotland (PIK).

Flexibility
Not every corner of the nation is glowing with radar coverage and saturated with VOR coverage.
Sometimes you want the flexibility of flying VFR with the ability to remain IFR. It's the best of both worlds.

There are two different operations:
1) VFR-on-top
2) VFR over the top.
The first one is an IFR clearance where you must maintain VFR cloud clearances.
