To keep it simple and short, do you need a 91 LOA to cross the Atlantic while staying below FL285?

If not, any links for reference would be great.

How long should it take to get a 91 LOA for MNPS Airspace once the process has been started? Aircraft already has an LOA for RVSM Airspace

Short answer is yes you need MNPS LOA for crossing even using the Blue Spruce routes. The time frame depends on your equipment and HF requirements.
 
MNPS airspace is between FL285-FL420. Ferried a couple F70's back-and-forth across Atlantic using Blue Spruce route, equipped with standard NAV systems, and no HF......non-MNPS and no issues. We did obtain a ferry permit to cover the lack of overwater certification.

Are you equipped with 8.33 radio(s)? That may severely limit your cruise altitude in Europe ( if you're flying something larger than a small GA airplane).

We flew Eastbound above the tracks at FL430 and came home below at FL270 with no MNPS LOA in a Gulfstream. There are no guarantees they will give you FL430 but traffic permitting they will give it to you.
 
Took us almost a full year to get all of our LOAs and that was for a brand new G550.

FSDO takes their sweet time to do anything.

I got given an older Falcon 50 on May 5th, un-announced. It went to Cannes via MNPS airspace with all the required LOAs on May 13th. Yes both dates were in 2014.
