You only cross the fix twice. First time it is an IAF, seond time an FAF.

No need to do an actual lap in the "hold' because it is not a hold, it is a racetrack course reversal.
 
How is the 090 radial between the 095 and the 238?
In my dyslexic world, not only is it between those numbers, but it's also the right staff, where dog is my copilot, and ducks do indeed have handlebars.

For the rest of the world, however, nosehair is correct...course reversal is mandatory. For everyone else, there is mastercard.
