5) 50 question ATP test and interview in FLL.
1a is technically not required but seems to improve the odds of making it to step 2 tremendously.

I'll third that recommendation. Dr. Walker knows his stuff and after a quite interesting year (medically speaking) I was happy to pay a bit extra to make sure the T's were crossed and i's dotted on my 8500.
